Possible Complications After Thyroid Surgery
Thyroid surgery is generally safe, especially when performed by an experienced thyroid surgeon. However, like any operation, certain complications can occur. Most are temporary and can be treated effectively.
1. Voice Changes (1–5%)
Why it happens:
The nerves that control the vocal cords lie very close to the thyroid. These may be involved by the disease or may get injured or pulled while performing the surgery.
What patients may notice:
- Hoarseness
- Weak voice
- Voice fatigue
- Difficulty speaking loudly
Treatment:
Most voice changes are temporary and improve within days to weeks. Voice therapy, nerve monitoring during surgery, and medications may help. Permanent voice change is rare.
2. Low Calcium Levels (Hypocalcaemia) (5–20%, usually temporary)
Why it happens:
The parathyroid glands, which control calcium, can become temporarily stunned during surgery. The chances of this happening increase if central compartment nodes are present or if disease is bulky.
What patients may feel:
- Tingling around lips or fingertips
- Muscle cramps
- Feeling of electric shocks in hands or feet
Treatment:
- Calcium and vitamin D supplements.
- Symptoms usually resolve within a few days to weeks.
- Long-term calcium issues occur in 1–3% of patients.
3. Bleeding or Neck Swelling (1%)
Why it happens:
Bleeding can occur in the first few hours after surgery.
Symptoms:
- Sudden neck swelling
- Difficulty breathing
- Feeling tightness in the neck
Treatment:
Immediate evaluation; in rare cases, a quick procedure may be needed to remove the collection.
4. Seroma
- Refers to fluid collection in the operative bed, majority of times it is harmless.
- It may present as a swelling in the neck.
- It may need aspiration with a needle.
5. Scar-Related Issues
Most patients heal well with a fine, thin scar.
Some may experience:

Treatment:
Silicone gel, scar massage, and in some cases, dermatology treatments.
